Notice: Initial Appointment Deposit

Please be advised that an initial appointment with Dr. Cruz requires a deposit of $68. This goes towards your initial exam fee. Your appointment time is reserved for you and you only. Additional scheduling may be done online. If you do not show to your appointment or fail to call/text/email that you cannot make the scheduled time within 24 Hours your deposit it forfeit unless rescheduled.

Your appointment time is reserved just for you. To better serve our patients and ensure that they have a fair opportunity to have an appointment as promptly as possible, we require a minimum 24-hour notice to cancel or reschedule appointments.

NO SHOW POLICY: Patients who fail to show up to an appointment and have not contacted our office are considered a “No Show” and will be charged the full cost of the appointment for each missed appointment.

LATE CANCELLATION POLICY: A “Late Cancel” is less than 24 hours notice. Patients who fail to cancel or reschedule their appointment within 24 hours will be charged half of the appointment cost.

As a courtesy, our system will send an appointment reminder 48 hours before by email and 24 hours then 3 hours by text message, unless you have opted out of these services.
